Can cross-industry principles help NHS mental health services?

For this post and all others (past and present), by psychiatry I also mean mental health services. Whilst psychiatry is a medical discipline it sits within and is normally closely linked to other aspects of mental health services.  Introduction In this post I explore how mental health services can learn from disaster management, mistakes, and failures across other industries. I will be thinking about what can be learned from the perspective of NHS psychiatric services. I often study many videos ...

Radical Transparency – is it for everybody?

The term sounds great, but what is it? Who invented this? Can we have it in the NHS or any other public sector service in the UK? This and more will be explored. Radical Transparency Radical Transparency is a management and organisational philosophy that emphasises open, honest, and direct communication within a company. It was popularised by Ray Dalio, founder of Bridgewater Associates, one of the world’s largest hedge funds. At its core, Radical Transparency involves encouraging employees at all ...

Beyond Nuclear Shadows – Navigating Trust and Secrecy

I only found out today about this major cover up in the history of the world.  I’m like “How on earth can one cover up a major nuclear disaster?!” But then my mind immediately drifted on to cover-ups in health services. If ‘they’ can cover-up a nuclear disaster in Russia, it shows the might of political forces. Some do not believe that a major cover-up could happen in UK health services. I’m not so sure. Enough distraction or lateral-thinking, if ...

Materiality

The case of Montgomery v Lanarkshire Health Board [2015] UKSC 11 is a landmark decision in the UK law of medical negligence, specifically in relation to the duty of a doctor towards a patient in relation to advice about treatment. It fundamentally altered the legal approach to patient consent and disclosure of risks by healthcare professionals. Materiality explained The concept of materiality was brought to the forefront of UK medical law in Montgomery. The Supreme Court held that a risk ...

Medical Practice Reimagined: Why ignoring the GMC’s 2024 standards could be a mistake

Introduction In the inner sanctums of the UK’s medical community, the words ‘General Medical Council‘ (GMC) often resonates with an undercurrent of apprehension, scepticism and cynicism. These sentiments were born from years of navigating the complex interplay between rigorous professional standards and the harsh realities of medical practice. For many seasoned doctors, the GMC is perceived not as an ally but as an imposing entity, distant from the daily grind of patient care and quick to impose what seems like ...

The vexed problem of collective intelligent output in team dynamics

Have you ever wondered how large or small groups of intelligent people on occasions end up making suboptimal or even stupid decisions? If you have, then this post may interest you. Importantly I said ‘on occasions’. That means that I am NOT saying that all or a majority of groups of intelligent people make stupid decisions. Scope and limitations I will drill into factors that affect groups where their collective intelligent output seems less than their average. I think this ...

When the GMC comes for you

I can tell you from real professional experience across the last 15 years that: Caution: I said the above was my experience and I am being 100% honest in my estimates on that. I do not know if my estimates apply to a total population of all doctors in the UK. I read parts of three key guidance documents (of 34) almost every day over the last 8 months. Over the last 5 years on average I would have read ...

Managing tail risk in psychiatry – collaborative mapping

Too often in psychiatry we are faced with the problem of tail risk. The basic idea is ‘low probability high impact‘ events. This commonly presents among relatively stable patients in contrast to patients in an acute phase of disturbance.  Some patients may have an established pattern of only one or two short-lived periods of disturbance per year where they present risks of suicide, homicide or violence, or arson. In any of those short-lived blips it is possible that serious harm ...

Systemic disempowerment in care-delivery organisations

Today I explore the term ‘systemic disempowerment‘ (SD) as pertains to large health and social care services. Health services are without doubt inseparably intertwined with social care services. The concept of SD moves beyond ideas of learned helplessness, which is so often thrown around. The concept of SD is far wider and deeper as this exploration will show. There is no easy way to make this blog readable in the desirable 30 second read; now widely popular among social media ...

Controversy – the God Complex

This is or will be a controversial exploration. Keep that in mind at all times. I boldly go where no psychiatrist has dared to go before! I give thanks to KJ for stimulating this exploration. What is the God Complex?  The God complex has also been referred to as ‘The God Syndrome’. Here are some descriptions of what it is and is not: The God complex is not a clinical term, nor does it appear in any diagnostic manual. The ...

Why lessons are not so easy to learn

The context of this exploration is twofold a) to examine how lessons may not be learned by individuals and b) difficulty for organisations in learning lessons. Repeatedly many UK organisations resolve to learn lessons. However the evidence shows that they are not learning lessons. What ordinary people expect Let us start by understanding how ordinary people might decide what is a lesson learned. I suggest that they may expect the following: The deeper reality Determining if an individual or organisation ...

Systemic failures and consequential risks

Conceptual overview: Systemic failures refer to breakdowns that occur within an interconnected set of components or systems, where the malfunction of one element can lead to cascading failures across the entire network. These are not isolated incidents but are often the result of complex interactions between various subsystems, policies, and human actors. The embedded YouTube video contains much of the text in this blog, and may be easier to process.  Introduction The National Health Service (NHS) stands as a beacon ...

From clinical to Institutional Psychopathy

Further to my recent post ‘Untangling a confused mess: motives and psychopathy‘, I dig deeper and extend further. This post does not mean that any organisation I know or have been connected to is Institutionally Psychopathic. I separate out dysfunctional organisational behaviours, then later in the post I bring them back under the umbrella of Institutional Psychopathy. In 2010 I coined the term Institutional Psychopathy. Evidence in screenshot below. At that time there were no publications containing the words together. ...

What do urgency and immediacy mean?

If someone is actively slashing their wrists or about to jump from a building, or about to stab someone to death – those are obviously urgent situations. The risk, the evidence and the dire consequences are easy to spot. There is a difference between matters that need immediate attention and those that need urgent attention. All matters that need immediate attention will be considered urgent. However, not all matters that are urgent need immediate attention. The infographic below aims to ...

Untangling a confused mess: motives and psychopathy

Who is a psychopath? The short answer to is NOBODY. How shocking will that appear to the general public who are programmed by social media, BigMedia and popular psychology websites? To learn more, you’d have to study from the facts and my reasoned opinions below. At first I will spend a few minutes demolishing the nonsense spouted by Raj Persaud. Then I will move into the concepts of motives, psychopath and psychopathy. I am not here to diagnose or psychoanalyse ...

Dream or reality?

In this article I focus mainly on public sector health care consultations. It is an open ‘secret’ that UK public health services are: short of staff, doctors are overworked, many groups of staff are suffering with burnout, clinic sessions are quite time-limited, follow-up visits are few and far between, patients are often not given sufficient time and support to ask questions. Following a concentrated first consultation for between 45 to 60 minutes, it is difficult for patients to remember all ...

The Lucy Letby story

Lucy Letby, 33, was convicted of the murder of seven babies and the attempted murder of six new-borns at Countess of Chester Hospital in 2015 and 2016. Why is this ‘story’ important for psychiatry? It shows how systems are slow to recognise and respond, slow to heed expert insights from within organisations, and how dysfunctional organisational behaviours contribute to harm and death. In my comparison of Surgical v Psychiatric Malpractice I showed how psychiatric malpractice is more difficult to recognise ...

Prescribing scenarios and the power of medications

In this post I will spend some time understanding the power of medications, then extract some fundamental principles from the GMC’s guidance on prescribing (April 2021). I then analyse two real-world scenarios in prescribing that I have experienced and reference each against the GMC’s guidance. [The scenarios do not contain information that identifies any patient. Both scenarios are common in medical practice] A patient requests a medication to treat side-effects of another medication (clozapine) that he is established on. This ...

What exactly is support?

Over the years I have come to appreciate that patients in psychiatric services very much value support. I’ve been wondering ‘What does support mean?‘ Hold on – I know that everybody knows support when they feel it. That would not inform me what aspects of support make it real or valid. Mental health workers and services without doubt need to provide support. Support can vary across various types of activities e.g. emotional support, advice, guidance, therapy etc. That does not ...

Surgical and psychiatric malpractice compared

As a result of recent media publicity about surgeon’s performance, I began thinking that I rarely if ever see any stories in the media about psychological harm caused by failures in psychiatric treatments. I reflected on this and thought, that with surgery it is easier to spot errors in retrospect because patients can be subject to x-rays, scans and other tests to show physical things that went wrong procedurally. But in psychiatry where we use medications (or  occasionally ECT), there ...

The relevance of culture in mental health services

I imagine that most people would begin thinking of the influence of culture on expression of signs and symptoms among mental disorders. However, there are wider areas of importance. What is culture? The concept of ‘culture’ is broad and multifaceted, encompassing a wide range of ideas, practices, and societal norms. The main concepts that comprise culture: Shared beliefs and values: Culture is often defined by a shared set of beliefs and values that guide behaviour within a group. These beliefs ...

Fundamentals of psychiatric diagnosis

Back in the 1990s, diagnostic criteria existed for sure. We had ICD-9 as our main diagnostic manual. My recollections from around that time was that the criteria were hardly ever referenced. How it worked was, the consultant psychiatrist would declare diagnosis or differential diagnoses without actually stating which criteria were met from available evidence. As a junior trainee, I had no say in the matter. In one adolescent psychiatry service around 1993, the consultant psychiatrist and a consultant psychologist stated, ...

Sitting in the lap of Big Pharma

Following a recent BBC documentary on antidepressants(2023), I was set on a path of deeper inquiry about psychotropic medications and the relationship of Big Pharma to psychiatrists as a group. Then my colleague S.D. got me reading: The Empire of Pain: The Secret History of the Sackler Dynasty. I was driven to do some digging about Big Pharma’s ‘offences’ in general and then sieved the list down to psychotropic medications. Note our disclaimers. This information is gathered from the public ...

Implications of the Dunning-Kruger effect

Through much experience over the last 30 years, I came to realise two important things: a) at times I made mistakes in exceeding my knowledge and competence b) in recent years I’ve seen others doing the same. The big problem is that one does not have an idea when one is stepping into territory that exceeds one’s competence. It is like a novice swimmer not appreciating the dangers of swimming into a wide open ocean. After all, one cannot know ...

Change management

Over the last 10 years I’ve visited for work, many public sector health services. Whilst no service is perfect, at each place I found things that were fundamentally wrong. Nearly every senior manager knew about the things that were wrong but they were unable to implement changes. I am referring to things that were known as wrong for years – not just weeks. The topic of change management is not new to me. My reflections led me to dig deeper ...

Followership

Not so long ago there were stampedes of people rushing towards ‘leadership’ courses. Leadership Universities were set up. MBAs and other management qualifications were in high demand. All courses emphasised leadership in change management. In many health services a culture was promoted that ‘everybody’ had to lead, though it was never stated like that. The stampede going into and out of courses, led (from my observations) to people in teams all trying to lead. I could see that failing at ...

Time management chaos or science?

Health service work involves complex and competing demands on time. No one should be surprised by that. Psychiatrists are often given some time management strategies to work with in their basic and higher training. However, my experience over the last 20 years is that most of those strategies fail. The reasons are: Excessive workloads Increasing demands for quality based on a number of relevant standards (in the public domain). The frequent unexpected number of urgent and complex situations that turn ...

Accountable health services

All public health services are accountable in the UK. This post provides an in-depth exploration of patient rights and accountability within the National Health Service (NHS) in the UK, focusing mainly on services in England. It delves into the sources of legally protected rights for patients, the mechanisms for holding the NHS accountable, and the challenges in upholding standards. The article highlights the importance of patient involvement in healthcare and outlines various ways patients can become more involved. The role ...

Hysteria, physician assisted paroxysms and sexual relations

This post is an off-shoot from Strange Medical Treatments in History. Introduction I began digging deeper into many of those treatments but then got stuck on what was termed ‘Physician Assisted Paroxysms‘ (PAP). I became totally flabbergasted by what I found. It would have been too much to add to that post, so I decided to pull all that I could find into this one. In essence PAP was actually and originally manual masturbation of the clitoris. That was seen ...

Strange medical treatments in history

This post was inspired by a non-medical colleague – initials L.R.  See Hysteria, physician assisted paroxysms and sexual relations which follows up on this post. The practice of medicine is built on ‘belief’ in treatments. History shows us how misplaced beliefs led to some horrific treatments, mistakes and death. Many of the treatments now considered “weird” or “outdated” were conceived and implemented in an era before the establishment of rigorous scientific methodologies that are now a cornerstone of modern medicine. Here ...

Reading the records

The delivery of healthcare is complex business.  Often times records are compiled by large numbers of people in teams or extension of teams about a single patient. The vast majority of Electronic Health Records (EHRs) are text-based. Understanding a patient’s condition(s) and needs from records is often demanding of time, effort and concentration. Many public sector healthcare organisations demonstrably fail to factor in the time that workers should spend reading the records. Time spent documenting in records is a separate ...

Caring explored

I was thinking about caring in the context of healthcare (only) for this exploration. There are some elements of care that involve providing services – for example diagnostics, investigations, nursing care, medical procedures and so on. But I needed to look deeper than that. I did not really want to see care as just services and jobs. Anybody could find a list of things that carers need to do to provide care e.g. risk assessments, filling in forms, taking pulse ...

Communication

Communication is a broad and multifaceted concept that can be defined in various ways depending on the context. Communication in health services is a very big issue. Numerous inquiries have found communication problems.  NHS trusts and other health service organisations are unaware of the costs of communication deficiencies. It is a very difficult thing to assess the cost to their business and the costs to patients. The following general definition will be adopted for this article: “Communication is the process ...

Experts and expertise

Preamble In 2012 I explored the nature of expertise in an article that was mainly focused on expert witness expertise. A deeper exploration was motivated today when I discovered a statement made by the Care Quality Commission. Scroll down for more. This article is not simply a rant about the CQC.  Expertise refers to a high level of knowledge or skill in a particular field or area. Experts are often recognised for their ability to understand complex information, solve problems, ...

Documentation

All health service personnel who deliver or are involved in delivering frontline health services are required to document their work. This is must be at the core of all health care. Follow-up topics: The Documentation Dilemma (Aug 2024), High level documentation practices (Oct 2024) and Decision-making (Oct 2024). What does documentation mean?  Documentation as a word has several meanings depending on the context in which it is used. Here are a few common interpretations: Documentation in health services Of the ...

Polypharmacy

Polypharmacy is the use of multiple medications by a patient, particularly when too many forms of medication are used by a patient, when more medications are prescribed than are clinically warranted, or when all of an individual’s medications are not clinically necessary. While there is no set number of medications that defines polypharmacy, the term is often used when a person uses five or more medications concurrently. However, polypharmacy can be present for someone on three medications, depending on the ...

Health debt – the post-pandemic trauma

The delay or disruption of healthcare services due to the COVID-19 pandemic has created what some have referred to as a “healthcare backlog.” This refers to the accumulation of health needs that were unmet or postponed during the pandemic, which now need to be addressed. See also pandemic lash. This backlog includes everything from delayed or cancelled elective surgeries to missed routine screenings and vaccinations, postponed mental health treatments, and disruptions in chronic disease management. These delays can have serious ...

What on earth is quality?

Quality is a multifaceted concept that can differ greatly depending on the context, product, or service in question. There is not a universal set of quality components that apply equally to every single product or service, but there are common elements that many quality evaluation systems tend to consider.[AC_PRO id=913] Key aspects of quality I outline some of the key aspects that often factor into the quality of both products and services. Performance: This is perhaps the most basic element ...

Pandemic lash on mental health

This article will deal with the impact of the COVID-19 pandemic on mental health. It is important at the outset to briefly review how the pandemic was realised, appreciate the depth and breath of the impact on health services in general – then drill down into what happened with mental health. I will focus on what economic stress may mean for the future. This post is a foundation for the follow up post: Health debt – the post-pandemic trauma. A ...

Attention Deficit Hyperactivity Disorder

ADHD affects both children and adults, and it can interfere with or reduce the quality of social interactions, school performance, and work performance. For something exciting skip to ADHD Scam Clinics. Introduction ADHD is characterised by three main groups of behaviour symptoms: Diagnosis is not simply a matter of checklists. Dealing with burnout

Burnout is a psychological syndrome that results from chronic workplace stress that has not been successfully managed. One of the first issues is ‘stress’. Not everybody feels it in the same way. Not all stress is bad. People have difficulty being aware when they are under abnormal stress. This article was followed by Getting serious about burnout: resilience has its limits (June 2024). Medication reviews

In the last 3 years I’ve received many requests for medication reviews. Interestingly most of those requests came from nurses; not pharmacists, not social workers and not other doctors. Underlying the requests were generally some concern that patients were not improving. Not a single request was about medication toxicity, or polypharmacy. Nurses tended to hint at the patient requiring higher doses or demand prescribing more medications. That body of experience led me to investigate the concept of medication reviews. All ...

What is antipsychiatry?

It is important in the medical practice of psychiatry to understand forces opposed to psychiatry. From my experience, I have witnessed antipsychiatry views even among trained mental health professionals. I declare a professional bias that I have not found those views helpful to patients’ progress. Introduction Antipsychiatry is a movement and perspective that fundamentally challenges the assumptions, concepts, and practices of mainstream psychiatry. It arose in the 1960s and has evolved over the years, with variations in its scope and ...

What do you know about psychiatry?

What is psychiatry?  Psychiatry is a branch of medicine that focuses on the diagnosis, treatment, and prevention of mental, emotional, and behavioural disorders. A psychiatrist is a medical doctor who specialises in mental health, including substance use disorders. Psychiatrists are qualified to assess and treat both the mental and physical aspects of psychological problems. Psychiatrists do not always use medications to treat mentally disordered people. People seek psychiatric help for many reasons. The problems can be sudden, such as a panic ...

What exactly is aggression?

Unfortunately, there is no exact definition of aggression. But that’s the beginning of this exploration, rather than the end of it. Aggression is realised when it is witnessed by observers. By contrast a person may feel aggressive but display no aggression. Disorders of intellectual development

Preamble The important words here are ‘intellectual’ and ‘development’. Many ordinary people are aware of a concept of delayed or arrested cognitive development that is referred to as learning disability. This article will abbreviate ‘disorders of intellectual development‘ to DOID. This article is not about acquired intellectual disability but some of the issues may apply. There is a tendency in an increasingly ‘woke’ environment of media and social media, to refer to these things as ‘issues’ rather than ‘problems’. I ...

Off-label and off-licence prescribing

In the UK, the terms “off-label” and “off-licence” prescribing are often used interchangeably, but they refer to slightly different concepts. The General Medical Council wisely unified both concepts to the term ‘unlicenced prescribing’ . It does not mean ‘illegal prescribing’. Most unlicenced prescribing in the practice of medicine happens in psychiatry. Why is unlicenced prescribing a big deal? In a nutshell, it is because when a medication is used outside of it tested development parameters special precautions ought to be ...
